ABSTRACT |

Posterior open bite has several possible causes. including primary failure of eruption (PFE). Primary failure
of eruption is a rare condition where there is eruption failure with no obvious cause. First and second molar teeth are most commonly
affected. Affected teeth will not respond to orthodontic force. Therapy of patients with this condition is extremely difficult. Two
methods of moving unerupted teeth into occlusion have been reported: to reposition them surgically by small segment alveolar osteotomy
or reconstruction of occlusion by prosthetic treatment. This clinical report describes a case of primary failure of eruption with
mandibular prognathism. The patient underwent a Le Fort I osteotomy and sagittal split ramus osteotomy. The occlusal plane was controlled
by orthodontic surgery. A good occlusion and facial esthetics were achieved, and these results have been maintained for one year
after completion of active treatment. This case report shows the effectiveness of reconstruction of the occlusal plane by orthognathic
