ABSTRACT
Guidelines for the prevention of deep vein thromboembolism (DVT) were introduced at Ichikawa General Hospital, Tokyo Dental College in April, 2004. From January 2002 to March 2004, there were 4 cases of perioperative pulmonary thromboembolism (PTE) out of 4810 surgical cases. After the introduction of the guidelines, there were 2 cases of PTE out of 8614 surgical cases until March 2008. No significant change was observed in incidence of PTE between these two periods. In spite of using a DVT prophylaxis based as indicated in the guidelines, PTE occurred in 3 patients, 2 perioperatively and 1 after cerebral angiography. From close examination of these 3 cases, it appears to be difficult to eradicate this problem. Therefore, we emphasize the importance of a scheme for early detection of perioperative PTE, followed by early diagnosis and early treatment. The incidence of perioperative PTE in dental and oral surgical patients is much lower than that in overall surgical patients. However, given the high mortality rate of PTE. we consider it important to screen out high-risk and highest-risk patients and to carry out appropriate DVT prophylaxis.(The Shikwa Gakuho, 108 : 627-631, 2008)
