ABSTRACT
The patient was a woman aged 36 years 11 months who visited our hospital with the chief complaint of protrusion of the upper frontal teeth. The bilateral lower second premolars had been lost. and molar conditions were Angle class I, + 14. 2-mm overjet, 0-mm overbite, and ANB 9.5º U 1 to FH 134º on cephalometric analysis. Maxillary protrusion accompanied by labial inclination of the upper frontal teeth was diagnosed. Extraction of the bilateral lower first premolars, removal of the pontic of the right lower second premolar, and orthognathic surgery by anterior maxillary alveolar osteotomy were planned. The bilateral upper first premolars were extracted, and leveling was performed, attaching sectional arches to the frontal tooth and molar regions during preoperative orthodontic treatment, followed by anterior maxillary alveolar osteotomy with interincisal osteotomy. Postoperative orthodontic treatment was performed, and the appliances were removed after dynamic orthodontic treatment for 2 years 2 months. Continuity of the dentition in the divided maxillary region and shortening of the treatment period were achieved, and a favorable facial profile and appropriate overbite and overjet were acquired.(The Shikwa Gakuho, 109 : 301-309, 2009)
